Clinical Social Worker-Social Worker

Job Description

Assist patients, families, and significant others with psychological, social, environmental, financial, and emotional challenges by conducting psychosocial assessments and providing appropriate interventions to enhance patient quality of life.

Key Responsibilities

• Manage daily workload allocation within the specialist area, considering competing demands and prioritizing caseloads

• Oversee and supervise junior staff, ensuring adherence to rules and policies

• Conduct comprehensive person-centered psychosocial assessments

• Plan and implement individualized care strategies in collaboration with patients, caregivers, and multidisciplinary team members

• Maintain and update patient records accurately, in accordance with JCI standards

Job Description

The Occupational Therapist will assess, plan, and implement specialized rehabilitation programs for patients with diverse or complex conditions, aiming to maximize their physical, psychological, and social functional potential.

Key Responsibilities

• Conduct comprehensive and accurate assessments of patients’ physical, psychological, and social needs, including complex and multi-pathology cases

• Develop individualized management and treatment plans using advanced clinical reasoning and a wide range of therapeutic techniques

• Evaluate patient progress and adjust treatment plans as necessary

• Manage own caseload under the supervision of a senior Occupational Therapist, handling referrals according to departmental policies

• Use verbal and non-verbal communication methods to effectively communicate diagnosis and treatment plans to patients and caregivers

• Develop discharge plans and coordinate ongoing care with relevant professionals

• Maintain accurate records, reports, and statistics in line with service policies and professional guidelines

• Stay updated with clinical and professional knowledge to maintain competency in assigned specialist areas

• Assist in maintaining adequate inventory of materials and equipment required for patient care

• Adhere to safety, health, and quality control procedures

Qualifications

• Minimum 2 years post-graduate experience as an Occupational Therapist in acute care hospitals or rehabilitation facilities
